Team 24 is organized and operated exclusively for charitable purposes in accordance with section 501(c)(3) of internal revenue code established in 2013. Our name originated from the 24 individuals that were in attendance at the first community meeting, hence Team 24. This journey was initiated by supporting local communities and now Team 24 is proud to be able to embrace the entire state of Arizona.

Team 24 is committed to inspiring optimism for future growth by partnering and working together with members of Arizona communities. Team 24 prides themselves on the dedication of supporting challenges that community members face by aiming to establish pathways that are practiced as a foundation to sharpened futures for community members. Team 24 utilizes resources in free educational services, humanitarian assistance, mental health services, social services, and much more to exceed these desires.

Gwen’s Advance Care’s Role
A Gwen’s Advance Care Support Coordinator has two functions; to be a “regulator” and “supporter.” Our contracts with providers ensure they are maintaining all licensing and recordkeeping requirements. We coordinate your necessary ongoing training and yearly renewal process. We will advocate for both the provider and the client (s) they serve by attending necessary meetings and by assisting in resolving problems. Home visits will take place a minimum of once per month. The decision to become a licensed Developmental Home provider should be carefully thought out. Please take some time to consider the impact that caring for another individual in your home will have. If becoming a Developmental Home provider seems too much of a commitment at this time, please consider other positive ways to make an impact on the lives of individuals with disabilities.
