Thank you for your interest in contracting as a vendor through Gwen’s Advance Care, LLC, as an Adult/Child Developmental Home provider. We take pride in supplying the highest quality direct care providers in the State of Arizona.1) The first step in becoming an ADH/CDH provider is to obtain an Applicant Questionnaire by contacting Ashanti Jefferson via email at [email protected], at (602) 595-5707 or by going on our website at www.gwensadvancedcare.com. 2) The second step is to submit the completed ADH/CDH Applicant Questionnaire via email to [email protected] or fax to (602) 595-7147. After it has been received and reviewed to determine availability in your area. You will be contacted to schedule a prescreening interview. You must complete this step before you can sign up for any trainings. We are contracted with the State of Arizona and most of our requirements are based on their mandates for being a certified provider. Listed below are the trainings that you must take in order to become certified in the State of Arizona through Gwen’s Advance Care, LLC. If you already have some of these items, please provide a copy of the certificate.
ADH
An ADH provider opens their home to provide a foster home setting to an adult or adults (18 and older) in need. ADH providers can support up to three adults in their home at one time. All adults placed in ADHs are receiving services through DES/DDD and have been diagnosed with a developmental disability. Developmental Disabilities can include, but are not limited to Cognitive Disability, Epilepsy, Cerebral Palsy and Autism. Adults placed in an ADH are usually long-term placements.
CDH
A CDH provider opens their home to provide a foster home setting to a child or children (0-17 years of age) in need. CDH providers can support up to three children in their home at one time. Most children placed in CDHs are receiving services through DES/DDD and have been diagnosed with a developmental disability. Developmental Disabilities can include, but are not limited to Cognitive Disability, Epilepsy, Cerebral Palsy and Autism. Children placed in CDHs can be temporary or long term, depending on the case plan of the child. As a CDH provider you are expected to follow and support the case plan of the child or children placed in your home.
Gwen’s Advance Care’s Role
A Gwen’s Advance Care Support Coordinator has two functions; to be a “regulator” and “supporter.” Our contracts with providers ensure they are maintaining all licensing and recordkeeping requirements. We coordinate your necessary ongoing training and yearly renewal process. We will advocate for both the provider and the client (s) they serve by attending necessary meetings and by assisting in resolving problems. Home visits will take place a minimum of once per month. The decision to become a licensed Developmental Home provider should be carefully thought out. Please take some time to consider the impact that caring for another individual in your home will have.
